I don't own a copy, but none of those I've seen had an insert..
About Godly Records, it's a fair guess it was a one-off thing..

The first OBITUARY-album also carries the "Godly"-label.

Yeah, it was Godly who were going to release it themselves, but then Roadrunner stepped in. Beep-beep.

I think there are no copies exclusively on the Godly-label, it was a cooperation from the start if I'm not mistaken. I guess Mr. Krgin had something to do with it.

It was Bori and Marty Eger's label. OBITUARY's LP never came out on Godly. Roadrunner released it from the start.
They were supposed to do a 2nd volume of R. DEATH with PENTAGRAM (chile), OBLVION.... but it never materialised.

I inquired recently about doing it on CD and learned a bit of history in the process. It's correct that Godly was a partnership of Borivoj and another person. Atheist were looking to use their tracks on reissues around 10 years ago, contacted Borivoj, who realized his partner couldn't be found. So all of the music was essentially just given back to the bands. With the exception of Obituary, whose tracks are owned by Roadrunner and were used on Slowly We Rot. The situation is basically the same with the Projections of a Stained Mind comp: the tracks are owned individually by the bands. To do a legal reissue of either would mean getting permission from a myriad of persons.
