Night on Brocken - I have the Metal Blade and the Roadrunner Versions both without lyrics
The Spectre Within - Roadrunner press with lyrics ! This is what my Database says, can check this evening if you like.
Perfect Symmetry - My copy is without Lyrics-Sheet

I checked my Fates Warning vinyls with the following result.
Night on Bröcken
Metal Blade
1st, 2nd (burning witch cover) and 3rd (1988 reissue, moon cover) editions all have the same lyrics and pics insert.
Roadrunner
1st edition no insert, plain white inner bag.
3rd edition lyrics and pics on inner bag.
The Spectre Within
Both Metal Blade and Roadrunner issues has got lyrics but with inverted color schemes (MB black on white, RR white on black).
Perfect Symmetry
Metal Blade with lyrics on insert - Roadrunner has no lyrics.
As for CD's I've little knowledge in how many pressings there are. I have the 1988 Metal Blade pressing of Night on Bröcken, the German 1994 Metal Blade pressing of The Spectre Within as well as the French 1992 Bröcken/Spectre double-pack (Metal Blade/Music for Nations) and all of them have the complete lyrics.

Of all i've seen recently on offer not even one has lyricsleeve or lyricsheet. Seems like all ex-owners took sheets out prior selling vinyls.

Nothing mentionned about Year or Matrix.
If it's bad quality and has a strange Matrix number could be a sign that it is in fact an Original from Azra !
Azra put out very few CD's and this wasn't one of them.
There are no legit CD pressings of "Ample Destruction" with that cover art.
The "No Poser Records" CD version is a bootleg as well.

LESE MAJESTY -Surviving
NASTY SAVAGE - Abstract Reality
LIEGE LORD - Burn To My Touch

The 7" only came with the original pressing (but I don't know how many...)

Yes. And as with the CD, the first one is CMFT only and the second pressing is CMFT/Metalcore.
Totally killer album though, no matter what label it is on...

Just got Omen - Battle Cry CD on MB / Restless, and the inserts are printed in quite bad quality. Also the design is odd, for example this is how the last page of booklet looks like:
Note the part at Steve, or the 71105-2 number above, or where Kenny's name starts on the left.
So my question: Is this a home made / reprint of a booklet, or did it actually come that way?

If I remember correctly mine is similar to this.
I don't recall the back insert being as bad as that though, but I don't have the actual CD in front of me to check right now.
I know that a lot of the early Metal Blade/Restless CD's were kind of tossed together quickly or had back inserts that were only slightly modified from the vinyl versions which didn’t always go over well in a small sized format so often they appeared very "home made".
I'll check and comment again later when I'm able to compare.
